What aftercare is and why it matters
Aftercare is the structured continuation of treatment after a higher level of care ends. Most relapses happen in the first 90 days after residential or PHP treatment, when a person is back in their daily environment but the changes from treatment have not yet fully consolidated into habit. Aftercare protects against that risk through three core mechanisms:
- Continued clinical contact with therapists and psychiatrists who already know the person’s case, tapering down in intensity rather than ending abruptly
- Relapse prevention practice not just learning skills during residential, but using them and refining them in real-world conditions with clinical support
- Connection to recovery community through alumni programs, peer support, and recovery groups (12-step, SMART Recovery, others)
Research consistently shows that engagement in aftercare is one of the strongest predictors of long-term recovery success. SAMHSA, NIDA, and major treatment outcome studies all identify the post-discharge period as the highest-risk window for relapse and the highest-leverage period for sustained recovery.
What aftercare looks like at Villa
Aftercare at Villa is built as a flexible continuum that adapts to where the client is in recovery. Most clients move through several aftercare components in sequence:
Step-down clinical care.
After residential or PHP discharge, most clients continue with structured aftercare services for 8 to 12 weeks, then to weekly therapy plus medication management. The same clinical team that worked with the client during residential continues during aftercare wherever possible, providing continuity of care.
Relapse prevention work.
Structured relapse prevention is woven into therapy and medication management. Sessions cover identifying personal triggers, developing concrete coping strategies, building accountability structures, and creating clear plans for high-risk situations. For substance use clients, relapse prevention also addresses managing post-acute withdrawal symptoms (PAWS), which can persist for weeks or months after physical detox is complete.
Medication management continuity
Psychiatric medications and medication-assisted treatment (MAT) for substance use disorder continue beyond residential. Many clients benefit from indefinite medication management; others taper off after sustained recovery. The decision is made between client and prescribing psychiatrist based on diagnosis, history, and goals.
Alumni and community connection
Villa’s alumni program provides ongoing connection through structured events, check-ins, and peer support. Many clients also engage with community recovery programs (AA, NA, Refuge Recovery, SMART Recovery) coordinated alongside clinical care.
Sober living when appropriate.
For clients whose home environment is not stable for early recovery, sober living arrangements are an option for the first 3 to 12 months after residential. Villa’s clinical team coordinates referrals to vetted sober living homes in the Woodland Hills, San Fernando Valley, and Los Angeles area.
Family involvement and education.
Family members are integrated into aftercare through family therapy sessions and family education. Recovery is more sustainable when families understand the diagnosis, the treatment approach, and how to support without enabling. See family therapy programs.
Aftercare Planning During Active Treatment
Aftercare doesn’t start at discharge it starts during residential or PHP. Discharge planning typically begins within the first two weeks of higher-intensity treatment and is finalized in the final weeks. Components of a Villa aftercare plan include:
Step-down level of care, therapy, or other clinical contact frequency
Medication plan ongoing prescriptions, refill schedule, psychiatric appointment cadence
Therapy plan weekly or biweekly individual therapy, with the same primary clinician when possible
Recovery community specific meetings, sponsors (for 12-step), or alternative recovery programs
Living situation home, sober living, or transitional housing arrangements
Trigger and high-risk situation plan specific people, places, situations to avoid or manage carefully in the first 90 days
Relapse plan what to do, who to call, how to step back into care if relapse occurs
Family and support contacts designated people who know the recovery plan and can support consistently
Work or school re-entry plan timing, communication, FMLA paperwork, accommodations
The aftercare plan is a living document. It gets revised as the client moves through aftercare and circumstances change.

Frequently Asked Questions
What is an aftercare program in addiction treatment?
Aftercare is the structured continuation of treatment after a higher level of care (residential, PHP) ends. It typically includes step-down clinical care like therapy, medication management, relapse prevention work, alumni support, and connection to recovery community resources. The goal is to support sustained recovery during the high-risk window after intensive treatment ends.
How long does aftercare last?
Length varies based on individual needs. The most intensive aftercare phase (IOP) typically runs 8 to 12 weeks. Outpatient therapy plus medication management often continues for 6 months to a year or longer. Many clients with co-occurring conditions benefit from indefinite psychiatric medication management. Long-term alumni connection is open-ended; many graduates stay engaged for years.
Does insurance cover aftercare?
Most major insurance plans cover aftercare services under the behavioral health benefit. Aetna, Cigna, Anthem Blue Cross, Health Net, MHN, and others are commonly accepted. Outpatient therapy, IOP, and medication management are routinely covered. Verification takes 15 minutes; call (818) 639-7160 or use the form.
Is aftercare different from alumni programs?
Aftercare is the clinical continuation of treatment therapy, medication management, structured relapse prevention. Alumni programs are the community connection side: events, peer support, and ongoing engagement with the recovery community at Villa. Most clients participate in both.
What is relapse prevention and is it part of aftercare?
Yes. Relapse prevention is the structured clinical work of identifying triggers, building coping skills for high-risk situations, creating accountability structures, and managing post-acute symptoms. It is integrated into IOP, outpatient therapy, and medication management throughout aftercare. Specific protocols include relapse prevention CBT, mindfulness-based relapse prevention, and substance-specific relapse skills work.
Do you provide sober living?
Villa coordinates referrals to vetted sober living homes in the Woodland Hills, San Fernando Valley, and Los Angeles area for clients whose home environment is not stable for early recovery. Sober living typically runs 3 to 12 months and provides structure, peer accountability, and substance-free housing during the most vulnerable post-residential period. Villa’s clinical team works with the sober living staff on coordinated care.
Can I do aftercare by telehealth?
Yes. Outpatient therapy and medication management are available by secure video across California, which is particularly useful for clients who have moved out of the LA metro area after residential treatment, or who cannot reliably commute weekly to Woodland Hills.
Do I need a referral?
No. Most insurance plans do not require a referral for aftercare behavioral health services, though some HMO plans do. Call (818) 639-7160 or use the verification form and admissions will confirm during the insurance check.
